The National Drug Code uniquely identifies a specific pharmaceutical product, including its package size. The NDC is required for each substance you submit to Far West Rx Disposal. NDC numbers are a ten digits, found at the top of the manufacturer’s packaging, and in the UPC bar code (which is twelve digits – ignore the first and last digits).
Ten digit NDC numbers will have one of these formats:
- 4-4-2 (
＊＊＊＊-＊＊＊＊-＊＊) - 5-3-2 (
＊＊＊＊＊-＊＊＊-＊＊) - 5-4-1 (
＊＊＊＊＊-＊＊＊＊-＊)
The only exception is for “generic” substance NDCs which start with G followed by 10 digits, optionally ending with two asterisks if there is no designated package size, e.g., G41870200**.

If you can not find the NDC number for the medication, please call the manufacturer you purchased the medication from and they should be able to tell you what it is. All compounds with controlled drugs usually have a bulk powder NDC code. This can also be found by calling the institution you originally purchased it from. Calculate the amount of controlled drug in the compound to figure out the transfer amount.
